2. Collect application forms available at NSSF-HQ for Tshs. 5000/
3. Return dully filled application form to the Fund
4. Memorandum and Articles of Association (For organization or company)
5. TIN/VAT certificate (For organization or company)
6. Certificate of incorporation/registration (For organization or company)
7. Four passport size of customer or one of the company director
